$1000 Sign on Bonus!


We are looking for a Care Coordinator with strong skills to join our Lewiston BHH team. You will work with the children of families within our catchment area based out of the Lewiston office which includes four counties. You will join a team of Care Coordinators, family support person, clinical lead, and medical/psychiatric/and nursing supports. At this time, much of the work will happen remotely due to COVID restrictions, however, some face-face time with families is allowed with precautions. We are looking for someone with the ability to manage a busy schedule, the needs of each family, and documentation needs.

This position also includes a $1000 sign-on bonus, $500 paid upon 3 months of full -time employment, $500 paid after 6 months of employment.

Duties:

Care Coordinators are the central piece to a network of providers working with each child. You will work to help coordinate and collaborate with each provider working with the child to ensure that their needs are being addressed - home needs, school needs, medical needs, and/or community needs. You will complete intakes for new clients, complete psychosocial assessments and other intake assessments, complete weekly documentation needs, make referrals, and complete initial and 90-day treatment plans. You will then continue to provide a minimum of monthly contact with each family and continue to collaborate with providers.

Qualifications:

BA in a related field. Good computer skills, and good communication skills. Must be well organized.
