Spurwink is hiring a school-based case manager/care coordinator to support students at Connors Elementary School in Lewiston. In this role, you will be serving youth identified as homeless, supporting their mental health, social, educational, and housing needs.

We are looking for someone with an ability to manage a fast-paced environment, has strong time management skills, a willingness to engage with community and school resources, and meet with families to support their child's overall health.

You will join a Spurwink team of Care Coordinators, family support persons, clinical leads, and medical/psychiatric/and nursing supports. You will also be working closely with Connors Elementary School staff and district personnel.

The position pays between $20.00-$23.50 per hour with opportunities for monthly and quarterly incentives ranging up to $400-450!

This position also includes a $7500 sign-on bonus, $3750 paid upon 3 months of full-time employment, $3750 paid after 6 months of employment.

Care Coordinators are the central piece to a network of providers working with each child. You will work to help coordinate and collaborate with each provider working with the child to ensure that their needs are being addressed - home needs, school needs, medical needs, and/or community needs. You will complete intakes for new clients, complete psychosocial assessments and other intake assessments, complete weekly documentation needs, make referrals, and complete initial and 90-day treatment plans. You will then continue to provide a minimum of monthly contact with each family and continue to collaborate with providers.


BA in a related field. Good computer skills, and good communication skills. Must be well organized.
